Refer Your IowaCare Patient Now

For your IowaCare patient, we have developed a way for you to refer them to Broadlawns Medical Center.

Refer to our “Guide to Services” to ensure Broadlawns offers a specialty.  Some specialties are limited at Broadlawns, not offered, or certain conditions are not treated within the specialty.  

For acute to acute or emergent referrals, please call the main number at Broadlawns and ask for the on-call provider at 515-282-2200.  Please DO NOT contact any provider directly.

Faxed referral forms are secure and may contain confidential information. We will follow up with you by telephone to obtain any information needed to complete the referral.

COMING SOON!  Specialty Clinics at Broadlawns will provide guidelines for an initial visit/consult.  The direct link will be posted here.  Check the site regularly for updates.  Please note that prior to scheduling an appointment, the specialist may require additional ancillary services the referring provider will be required to provide or coordinate.

Please use the form at the link provided above, along with the Release of Information.  If you have electronic capabilities, we still require the patient Release of Information.

Electronic referrals must contain:  specialty requested, reason for initial visit/consult, all required records and tests related to condition, and other pertinent information.  Inappropriate or incomplete referrals will be faxed back to the Medical Home.   

Please remember the following:

  1. We DO NOT provide radiology or lab services to secondary providers. There is a funding stream for this to contract with providers at the Medicaid rate.  Please contact IME, rather than Broadlawns, if you have any questions regarding this.
  2. Specialty referrals cannot be scheduled directly through the clinic or through the appointment desks.  Referral forms must be completed and submitted to the Referral Specialist.
  3. Specialty referrals must come directly from the patient’s Medical Home.
  4. Please send radiographs and other imaging via CD.  The Medical Information Management Department at Broadlawns is unable to store hard copies of radiographs and other images.

To reach a care coordinator immediately, please call the Broadlawns operator at 515-282-2200 and ask to have a member of the care coordination team paged during standard business hours.
