Under the supervision of the Clinics Administrator, the Case Manager coordinates the care of a patient with insurance, provider, state agencies, home health, other facilities, etc. The Case Manager maintains compliance with State professional nursing standards, The Joint Commission, insurance regulations and all Federal and State standards of care. Case Management is a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet the patient’s needs through communication and available resources to promote quality cost-effective outcomes. The Case Manager will perform a wide variety of high-level patient care activities to include Case Management assessment, screening for appropriateness of admission, order set monitoring, development of discharge plans and patient and family teaching for selected patients. Much of the Case Manager’s focus will be on patients with frequent hospital admissions and those patients with chronic illnesses
Generally Monday-Wednesday, 8am-4:30pm
Current RN license recognized by the State of Iowa Board of Nursing or Bachelor’s degree in Social Work with current Iowa license or Master’s Degree in Human Services. Minimum of two (2)-plus years of experience with hospital setting preferred. Experience with discharge planning. Strong professional level knowledge of comprehensive clinical assessment skills in the adult population and experience with chronic disease management preferred.
