Community Access Program (CAP)
Targeted Case Management
Targeted case management services link consumers with chronic mental illness to service agencies and support systems responsible for providing the necessary direct services. Targeted case management coordinates and monitors mental health and community support services through a team composed of the consumer, case manager and relevant providers and natural supports.
Supported Community Living
Supported community living services are provided to persons with chronic mental illness to develop a support system and learn skills that will enable them to live, learn, work and socialize in the community. Services are individualized, and need and abilities-focused. Supported community living services may be funded as community support services through the Iowa Plan for Behavioral Health (administered by Magellan Behavioral Care of Iowa), Adult Rehabilitation Option services through Medicaid, or by the state or county. Most persons receiving supported community living services have a primary care coordinator through targeted case management, service coordination or integrated services.
Service Coordination
Service coordination services are county-funded care management services for persons with mental illness or chronic mental illness who do not need targeted case management or integrated services. The county of legal settlement must agree to pay the monthly service coordination fee to the service coordination provider.
Positive Alternatives to Hospitalization (PATH)
Integrated Services Project
The lead agency integrated services projects provide county-funded care management services to persons with mental illness and chronic mental illness, who have no other insurance coverage. Integrated services are highly flexible and individualized due to the project design that is deregulated, decategorized and capitated.
This service or program is funded in part with Polk County funds.
