Advance Directives state your choice for health care treatment if you cannot communicate or make decisions. There are two kind of Advance Directives: Power of Attorney for Healthcare Decisions and Declaration About Life Sustaining Measures.
Power of Attorney for Healthcare Decisions identifies the person that you trust to make healthcare decisions for you when you are unable to do so for yourself.
Declaration About Life Sustaining Measures (Living Will) describes the type of medical treatment you want or don’t want at the end of your life.
For more information or to download the forms, visit The Iowa State Bar Association (ISBA) website. On the homepage, go to the search icon (magnifying glass at the top of the page) and type in "power of attorney and living will" in the search bar.
When you become a patient, you will be asked if you have an Advance Directive. If you do, please bring it with you to include in your medical record. If you do not have one and would like assistance completing an Advance Directive document, please contact the Chaplain at (515) 776-0535 or Case Management Services at (515) 282-6669.
