Social & Community Connections

Our health is shaped not just by what happens in a doctor’s office, but also by the people around us and the support we receive. Strong social connections and community involvement can boost well-being and lead to better health. When people feel isolated or disconnected, it can have the opposite effect.

How Broadlawns Supports Community Health

At Broadlawns, we understand that staying healthy often requires more than medical care. That’s why we team up with local partners to help individuals and families get the tools and support they need to live healthier lives. These partnerships connect our patients to housing support, nutrition programs, job training, transportation assistance, and more.

Programs That Make a Difference

Food RX Program: Doctors at Broadlawns can prescribe fresh fruits and vegetables as part of a patient’s care plan. Patients and their families receive $30 per person each month to use at local grocery stores and farmers’ markets. The program includes monthly check-ins for six months to support ongoing progress.

Health Coaches: Our Health Coaches work one-on-one with patients to support healthy changes—like starting an exercise routine, choosing healthier foods, or setting manageable goals. Coaching is available to Broadlawns patients of all ages, with no referral needed. It’s a personalized, practical approach to long-term health.

Community Vaccination Clinics: Each year, Broadlawns partners with area organizations to offer pop-up vaccine clinics across Polk County. These clinics provide important health services and education in neighborhood settings, making it easier for families to stay up to date on recommended vaccines.

During the 2024 fall flu season, Broadlawns gave over 700 free flu shots through five community clinics. In August, we held a Back-to-School Vaccine Clinic with DSM Refugee Support, helping nearly 30 children get the immunizations they needed to start school on time.
